Physical Evaluation Form
Instructions
A completed KSHSAA Pre-Participation Physical Evaluation Form is REQUIRED. The available form for download is the ONLY Physical Evaluation Form that will be accepted! Be sure you follow the enclosed instructions on the first page closely. A properly executed form must be uploaded and on file with the Andover Jr Sports Association before an athlete can be rostered to a team and any equipment or uniform checked out to an athlete!
Read the enclosed instructions first and Don't forget all required signatures!
The Physical Evaluation can only be completed and signed by a MD, DO, DC, PA-C or APRN.
The Physical Evaluation is required annually, and Physicals must be dated after May 1, 2024 to be accepted. The Physical is not required to attend Jr Football Camp but is recommended.
