Physical Evaluation Form
Instructions
The Physical Evaluation Form is mandatory and must be downloaded from the Resources Page. Please follow the instructions on the first page carefully. The form must be completed and signed by a qualified healthcare provider (MD, DO, DC, PA-C, or APRN) after May 1, 2025, to be accepted. A physical is not required to attend Jr. Football Camp but is strongly recommended.
Important: Physical forms can only be uploaded to the athlete’s registration profile after team assignments are made. Team assignments will begin in July, following the June 30 registration deadline. All pages must be scanned and combined into a single PDF file. Incomplete forms, missing signatures, unofficial versions, or illegible documents will not be accepted.
